Membership Terms

The "Insider" Membership Plan is limited to the following terms. Choice of ONE treatment package for the YEAR: Pick one of the following Neurotoxin (limit 120 units); Dermal Filler (limit 3mL); The ULTRA GLOW LaseMD Laser Resurfacing & Serum (limit 6 treatments); Kybella (limit 3 vials); The GOOD HAIR KeraLase Treatment (limit 4 sessions). Additional treatments not listed or above limit will be charged at the discounted rate of 10% off. The membership period is for a term of 12 months. Terminating the contract before the expiration date will incur an early termination fee of $1,000 that will be charged to the card on file. If you are unable to make it to your appointment, please cancel at least 48 hours in advance. If an appointment is missed or not cancelled within that time frame, you will be unable to rebook that treatment for 30 days, or forgo the membership discount associated with the appointment for 30 days. The "VIP" Membership Plan is limited to the following terms. Choice of one treatment per month for a year from the following list: Neurotoxin (limit 160 units); Dermal Filler (limit 5mL); The ULTRA GLOW Facial: LaseMD Laser Resurfacing & Serum (limit 6 treatments); The ULTRA FIRM Facial Genius Skin Tightening  (limit 6 treatments); Kybella (limit 4 vials); The GOOD HAIR KeraLase Treatment (limit 6 sessions) Additional treatments not listed or above limit will be charged at the discounted rate of 15% off. The membership period is for a term of 12 months. Terminating the contract before the expiration date will incur an early termination fee of $1,000 that will be charged to the card on file. Choice of one luxe treatment per month from the following list: Neurotoxin (limit 300 units); Dermal Filler (limit 7mL);  Ultraluxe Facial (limit 6 treatments), Kybella (limit 6 vials); The GOOD HAIR KeraLase Treatment (limit 12 sessions). Additional treatments not listed or above limit will be charged at the discounted rate of 20% off. The membership period is for a term of 12 months. Terminating the contract before the expiration date will incur an early termination fee of $1,000 that will be charged to the card on file.
